Medical Information

The health and safety of all students is a priority at Moonshot Museum.

Parents and/or legal guardians must disclose all medications their child may require during the conference on the required participation forms prior to the start of the session.

Students who require emergency medications, including but not limited to inhalers and/or EpiPens, must carry them at all times in a labeled waist pack and/or small backpack worn by the camper throughout the day. The bag must be clearly marked with the camper’s name.

Parents and/or guardians will be notified immediately if emergency medication is administered.

Students may carry and self-administer medication if they are capable of doing so independently. All medications, whether prescription and/or over-the-counter, must:

• Be in original packaging
• Be clearly labeled with the camper’s name
• Include dosage instructions

Prescription medications must also include:

• The prescribing physician’s name
• The physician’s phone number
• Clear timing and dosage instructions

Moonshot Museum staff are not responsible for storing, housing, and/or refrigerating medications.

If a student is approved to self-administer medication, a written statement from the child’s medical provider and/or parent or legal guardian confirming the child’s ability to self-medicate without assistance must be submitted prior to the conference.

Moonshot Museum staff do not dispense medication of any kind. In the event of a medical emergency, staff may assist with the administration of life-saving medication such as an inhaler and/or EpiPen, provided written authorization from the parent(s) and/or legal guardian(s) and physician documentation are on file.
To protect the health and safety of all students and staff, please keep your child home if they are experiencing any of the following:

• Fever, especially when accompanied by a rash and/or behavioral changes
• Vomiting more than once within the previous 24 hours
• Persistent diarrhea and/or frequent loose stools
• Open and/or draining sores that cannot be fully covered
• Worsening respiratory symptoms, including cough, sore throat, and/or congestion not related to allergies

A student may attend the conference when:

• They have been fever-free for at least 24 hours without the use of fever-reducing medication
• They have not vomited for 24 hours and are able to tolerate food and/or liquids
• Diarrhea has resolved for at least 24 hours
• Open sores are no longer draining and are under medical treatment
• In the case of strep throat, they have been on antibiotics for at least 24 hours
• Respiratory symptoms are improving overall for at least 24 hours

If a student becomes ill during the day, a parent and/or guardian will be contacted and must arrange prompt pickup.

Your cooperation ensures a safe, focused, and high-quality experience for every student at Moonshot Museum.

PARTICIPATION AGREEMENT, ASSUMPTION OF RISK, RELEASE OF LIABILITY, AND MEDIA CONSENT

By registering my child for the 3rd annual Jr. Space Conference operated by Astrobotic Foundation d/b/a Moonshot Museum (“Moonshot Museum”), I certify that I am the parent or legal guardian of the minor participant and have full legal authority to execute this Agreement on their behalf. I understand that participation in Jr. Space Conference is voluntary and that this Agreement is binding upon me and the minor participant.

I understand that Jr. Space Conference includes hands-on science experiments, robotics and engineering projects, electrical circuits, prototyping activities, art-based programming, use of tools and materials, and general supervised movement within the museum facility. I acknowledge that participation in these activities involves inherent risks, including but not limited to cuts, burns, falls, equipment-related injuries, allergic reactions, or other unforeseen injuries. I knowingly and voluntarily assume all risks, whether known or unknown, associated with my child’s participation in the conference.

To the fullest extent permitted by the laws of the Commonwealth of Pennsylvania, I release, waive, discharge, and hold harmless Astrobotic Foundation d/b/a Moonshot Museum, its directors, officers, employees, volunteers, affiliates, partners, and agents from any and all claims, demands, causes of action, damages, losses, or expenses arising out of or relating to my child’s participation in the 3rd annual Jr. Space Conference, including claims arising from the ordinary negligence of Moonshot Museum. This release does not apply to claims resulting from gross negligence or willful misconduct.

I agree to indemnify and hold harmless Moonshot Museum from any claims, damages, or expenses arising from my child’s actions, misconduct, or failure to follow conference rules and staff instructions.

In the event of illness or injury and I cannot be reached, I authorize Moonshot Museum staff to obtain emergency medical treatment for my child. I understand that I am solely responsible for any medical expenses incurred. I confirm that all relevant medical conditions, allergies, and health information have been fully disclosed during registration.

I understand that supervision is provided only during designated conference hours and that Moonshot Museum is not responsible for student outside official drop-off and pick-up times.

I understand that respectful and safe behavior is required at all times. Moonshot Museum reserves the right to dismiss a student whose behavior endangers themselves or others, damages property, or significantly disrupts the camp environment. I acknowledge that no refunds will be issued in the event of dismissal for behavioral reasons.

I understand that my child may be filmed, recorded, or photographed during conference activities. I grant permission for Moonshot Museum to use my child’s name, image, likeness, and/or voice in any format for lawful purposes including advertising, marketing, research, public relations, publicity, packaging, and promotion without further consent or compensation. I waive any right to inspect or approve finished materials and waive any claim arising from the use or editing of such materials.

If any provision of this Agreement is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect. This Agreement shall be governed by and construed in accordance with the laws of the Commonwealth of Pennsylvania and shall be interpreted broadly to provide a release and waiver to the maximum extent permitted under applicable law.

I HAVE CAREFULLY READ AND FULLY UNDERSTAND THIS AGREEMENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A LEGALLY BINDING CONTRACT, AND I AGREE TO ITS TERMS.
